How to get a catalogue entry in the SELECT @ Bath Catalogue

Please print out and complete the following form with your details. You will be listed alphabetically by the first letter of your company name. The chosen catalogue name will also appear on your shell scheme fascia nameboard.

Return by the 28th of March 2010

Please complete all sections. If you are sharing a stand, return one form for each company you require listed in the show catalogue.
